Check the Polarity of D+ and D-
For my application, the polarity shown in the pictures above were reversed. I struggled for quite a while before out of desperation I reversed the wires on D+ and D-. At that point everything worked properly. So I would say the pictures above are mis-labeled.

Works good with Valence batteries
Works good with Valence batteries. The shell looks "repurposed" and the activity LED is only visible because it illuminates the crack between the shell and the terminal block... but it works good.
