The cap does not post on the back of the pen, so be careful not to lose it. Unscrew the nib from the body of the pen. The cartridge goes inside the body with the small end toward the back of the pen and the larger end toward the nib. If you just place the cartridge inside the pen and screw it closed it will not automatically pierce the cartridge. If you want an extra fine line the Platinum Carbon Extra-fine writes a nice very thin line. It is sometimes listed as a superfine nib.
