There's a small hole on the bottom of the piece that fell off. Hopefully, you still have the screw in the hole. It's a small hex head. The hex key came with the original box. You can get a hex key set from Harbor Freight or any store. **Q: The description says this is polished chrome but the model number ends with the "-bn" suffix for brushed nickel. which is correct?** A: It depends on which item you choose. You can get either one. Go by description words not codes. One says polished Chrome and the other says brushed nickel in description. Choose what you prefer. **Q: I’ve only had it up for two months and the arm that swivels lost its little screw. How do I get a replacement?** A: I would contact the company. I can't believe they don't have replacement parts. **Q: We returned another brand as it wouldn't fit Costco tp rolls. Don't care for gap against vanity on pivot side. Anyone filed down mount to close it up?** A: I totally agree! I thought about filing it down but then when it pivots it will scratch the wall. POOR DESIGN!!! It looks very expensive and substantial. I also love how the left side pivots UP so you can change your TP rolls. Installation took me a little longer for this one compared to the other Kohler Alteo (robe hook and towel ring) items I purchased. Kohler's instructions are very generic as they use the same instruction sheet for many of their bath fixtures, so don't expect individual step by step instructions for the item you ordered. I also recommend using the anchors that are provided. I tried to install it without the anchors, and the screws didn't stay in place and were very loose in the wall.
