You do need a drill to properly anchor and screw the holder to the wall as well as a tape measure and level. Screws and an Allen key were included. The design does not make the toilet paper roll fall off! You can easily tighten the rod that holds the roll to any length and adjust to sit on the ledge. It is a nice quality and worth the $24, which is not expensive in comparison to the like counterparts.
