Can I just buy handle replacements?

I'm thinking that you could probably just remove the ones that are there and put pretty much any handle on there! Thing is, these are typically 'standard' size as far as the nut they screw on to. As far as handle replacements - Frankly, this faucet weighs practically nothing since it's predominantly plastic. It looks fine and as long as it's not in a bathroom that gets heavy duty use - well, you could fool people with it, but as far as replacing any parts? I'd give in and replace the whole thing with a better quality faucet! I have it in a 3/4 bath in a bedroom suite that it's used much...Short answer - probably no replacement by company, but you can replace with 'something'.

Can the knob come off to put an extender?

The end of the knob unscrews leaving about 3/4 inch of threads exposed

Where do the rubber washers go

Maybe you should have a plumber install although if you've ever done one it's not terribly difficult. An under sink wrench is recommended because no other wrenches will fit in the tight spaces.
