Are the stainless steel washers flat or conical?

Conical, will flatten once you torque them down

is this really 1/4th in diameter in the hole?

Yes... the metal hole is 1/4" in diameter. The rubber washer part makes the hole a smidge smaller. They work perfectly with roofing nail and exterior/decking screws.
