Depends on what you use it for. As a pillow it’s very safe.

I wake up with very shore neck and upper shoulders, does this pillow helps me with my issue. I have tried mypillow but didn't get any good result.

How to these hold up if you have cats that lay on them? I'm afraid of their nails...

They should be fine. There is a thick plastic bag (which holds the water) on the inside of the pillow and then "pillow " surrounds the bag. So they would have to have very long nails to get through...I would think. Its very sturdy to keep from leaking everywhere so I really cant see a cat's nails puncturing it.
