One review stated this was used to preserve a rust patina. Would this help automotive clear spray adhere and preserve the rust on an old milk can?

I only used this product to remove a lot of old rust on an outdoor iron rack. It didn't do the job. Don't have experience with the other product or method you are attempting.

Why is it "exterior only"? Can it not be used with interior oil based paints? (with proper ventilation, I assume the fumes are bad)

Yes you are correct. When spraying the oil based paint along with penetrol are a bit overwelming with fumes. I have used a mid priced automotive paint resporator to spray interior doors and trim. Remember that oil based paints are hard to clean up(more masking, longer cleaning time, more spray gun cleaning, solvents, and lots of basic throwing away of brushes, rollers, trays, drop clothes)

Can I use penetrol as a top coat over enamel paint?

I wouldn't. That's not what it's for. It's a paint additive to remove lap marks when the paint is still wet, and it works well as a way of preventing rust from further oxidation and keeping the patina of rust, but it's not a top coat the way that other clear coats might cover over paint.

Can this be used with water-based urethane alkyd enamel?

This is only for oil based paint.
