How can I attach the last piece that extends the height to the tripod?

The last piece you mean the ball head or center column? Either way, you just screw in. Unless you lost the little 3/8 to 1/4 adapter, then you won't be able to.

Which Carbon fiber extension rod will fit this tripod as it is apparently not included anymore?

ANY extension will work as long it has the same thread size on the mount screw..... just don't go TOO big as the tripod is not designed to be tall... so the taller it is, the less stable it is....

Would this handle a 4 lb load well?

yes as long as the camera lens doesn't make the tripod off center of gravity.
