Which is actual voltage it requieres to operate?

It operates from 5 Volts DC, but it pulls quite a bit of current. The Bias-T in my SDR (RSP2-duo) will not power it. I use an external Bias-T for power.

how far away from Smartee can this be placed and still be powered properly?

It depends on the cable type used, but very far in practice.

Does the "Input" end connect to the antenna?

The end with the mini-USB power connector connects to the antenna.
