It's a hanger. Round end goes around the hose so you can hang it from the stove frame for storage

Will the burner stay lit with a low flame? I need a burner for my smoker and it needs to maintain a low flame.

Can I add an extension to neck length (another 12")? Need to use in a clay oven and have the hose connection be outside of the oven?

No. it is all cast in one piece. You could add a pipe to the end of the hose but, then your air adjustment would not be accessible
