Q: Why isn’t the display backlit like in the pictures?

No Backlit display!!!! Omron drop backlit displays and really needs to rethink this decision. People with eye issues really need this capability.

Q: Will the omron wrist cuff read accurately if you have pvc and pac ?

This wrist cuff is not accurate at all in any circumstances. I found it consistently read high compared to my omron arm cuff. I talked with my doctor and she informed me that wrist cuffs are not as accurate as arm cuffs. The wrist cuffs are convenient but it appears they give false readings. Mine constantly told me I was in hypertension crisis and to go to the er immediately. I then used my arm cuff and found my bp to be perfectly normal. I'd discuss with your doctor to find what works best for you.

Q: Will this take readings as high as 200/100? The cuff seems to deflate without a reading sometimes

I have primary aldosteronism which is when you have high levels of aldosterone and little to no renin in your adrenal gland. That being said I can have very high readings if I forget to take my medicine (which has only happened once) which is not good at all. However, I have had an instance that my pressure was high with a reading of 200/104 and it clearly showed up on my monitor.

I did not experience e4 errors, but did find this unit to be terribly inaccurate so purchased the table model with arm cuff and it is much more accurate. I have found that transferring readings to my iPhone from either unit isn't the most straightforward process, but they do always eventually get transferred.
