Can a man in his 90's use this fairly easily?

I don't see how age would matter. It's pretty basic...as long as he can unscrew the top and get it to where it has to be, I don't see an issue with an elderly man using this at all.

Do you make a screw on adapter for females?

I never saw that it does...probably have to order a specific model for females....

Does it work while laying down?

Yes, that's exactly how it's supposed to be used after a person has surgery and can't get up to use the restroom. You roll on your side and insert your penis into the opening being careful to keep the opening raised above the rest of the urinal so that it doesn't spill. Usually it can be used several times before it becomes too full to prevent spillage. The neck and lid have large threads to make it easy to put on the lid and it's very secure. The urinal is designed to hang on a hospital bedrail but it also works on many/most normal bedrails. I shift the mattress over a bit to create room for the handle to slide over the bedrail easily.
