Q: do the jackets come in different sizes ?

They do but you have to search for your size as a different item, this one is for a medium. If you do alot of welding especially overhead this jacket might not be best. I've suffered 3rd degree burns to my neck, collar bone area, shoulders, and back. Because they're really not FR Jackets. Hope this helps.

Q: What size is MEDIUM? 38,40,42? Any ideas?

I am 5'9" and it is a little big but Medium was the smallest size. There are snaps on the cuffs so I am able to adjust it well enough to make it fit. I hope that helps.

Q: Is it insulated so it keeps out the heat? It gives the impression that it is too hot

Thin cotton material, is NOT insulated. If weld spatter lands on it, you know pretty quick. Good to wear while grinding though.
