Is this compatible with my S2001?

According to the Miele 2001 manual, this uses the SEB 217-3 OR SEB 228 which is the same as my Miele Titan uses. The SEB 217-3 was not to powerful and died on me so I replaced it with the SEB 236 and it is much better and stronger. Will this head work with the S5280?

Yes, It will work on any Miele Vacuum made in the last 20+ years.

What hose connection do I need with this to attach to C3?

You will need the electric hose and wand depending which unit you have.

Given a choice, would you still purchase the 236 and not the 228?

Yes.Pros of the 236: Better cleaning - It gets the dog hair up really well. The 236 was purchased to replace a 228. LEDs also let you see what you missed so you can take another run at it.Con: Heavy - The thing is like a boat anchor!
