Q: I wear size 5 underwear, what size is best?

To be honest these period are no good . You will bleed straight through them. Save your money look for something else

Q: Are they actually black on the inside? It looks gray from the review pics.

The absorbent part is lined inside and it is grey.

Q: Do you have to only hand wash these underwear or can they be put in a washer machine?

I hand rinse right after use and then throw them in the washer/dryer with everything else. I've had them a few months now and still no leaks.
