Can this tool be used to lift a full size 1/2 ton pickup bed?

yes, but i would throw away the chains that came with it, and the junk "shackle" the rest of the tool is fairly decent, but i sure wouldnt' trust it to 4000 lbs, maybe 1000 note that this tool doesn't do any actual lifting, it's only for tilting the load forward or backward while lifting with a cherry picker, crane, engine hoist, or similar

Does this come with the lifting chains? I see its the leveler. But does it come with the chains to attach to the engine

No. And I had to call Amazon and demand a refund. They said they would correct the product picture but, obviously, they have not.
