Can it be taken apart for transporting without removing the tv from the brackets?

Yes. Take the bolts out that are used to select the height of the TV and there you go. It will have the stem that fits into the upright still attached and might be a bit awkward if you are trying to pack. But you do not have to remove the TV from the bracket. Hope this helps.

Does the floor stand come with the hardware needed to mount tv to floor stand?

Yes, hardware to mount the tv is provided.

How easy is it to adjust the height? Are tools required to do so?

Pretty easy but you do need a screwdriver. But the screen tilts so you don't really need to adjust the height. At least I haven't at all since I've been using it and I initially thought that'd be a problem.
