How noticeable is this product if I wear it under my clothing while traveling? Especially thinner, women's summer clothing that is not very loose?

It's very noticeable. If you're trying to hide it, you're going to need to slide it under your arm pit, or wear a coat of some sort, which makes wearing it in summer a problem. I had to remove the slide because it was molded VERY sharply and cut into my neck. I also reduced the length of the cord because it was VERY long. Fortunately, I had an old slide so it was no problem.

Does the strap have a wire in it so their not able to cut it?

The strap is not wired. It is a woven material like a super heavy duty shoestring. It has a toggle device that allows you to adjust the length of the strap. You can wear it long or shorter.

What's the best way to clean this?

warm water and soap- I used dawn with some warm water to get some food off- I should have had it tucked under my clothing, but I dragged it through a plate of food accidentally. It cleaned up fine- I have the black color.
