Does this device attach firmly to the ball?

Yes, it attaches firmly and won’t come off until you pull it off. The ball can rotate with the device on, though, so it’s a good idea to hold it in place while marking the ball.

Does this allow 3 lines on golf ball?

You can make 3 long parallel lines by using the middle slot and carefully rotating ball in template. Otherwise, the 2 outside lines will have a break since there are 2 perpendicular lines on the template. Either way, good tool. I prefer to have the perpendicular line to help with squaring putter face to target line.
