was this height determined by veterinarians?

Difficult to answer because cats are different heights. Recommendations I saw indicated a body position of the head being aligned with spine rather then the head in a 'bent down' position. I needed to add another small dish on this table to make that body position happen for my cats. Still love the table-no slipping dishes and easy to clean.

can this be used alone as a water dish?

Sure, but it would be a very shallow water dish. As pictured the indention is only .5 to .75 of an inch. If you turn it over it is deeper, but there are several supporting bars that would make efficient cleaning a bit more difficult.

Can you stack these “tables” to create a higher feeding surface? My cat has megaeaophagus needs to eat upright.

Sorry, they do not stack. I already had another slightly elevated dish that fits on the 6 1/2" surface (with room to spare). Since both are plastic, I have a smaller glass dish that holds the food. With all 3, my cat's body is fairly straight from head to tail. Hope that helps.

can i use this without a bowl for dry food? I want a flat surface for my cat to eat from so she cant gorge herself.

Yes, it plastic, so you may want to verify that your cat is not allergic to it. Kitties can get sores around their mouths that are very painful from some plastics. The stainless steel dishes that fit these can also have food spread out on them to slow our fast eaters down.
