Question 1

What is the largest size that can be measured? Isn't a US size 5 for a 12-18 month old?

There is a toddler size 5 and a bigger child size 5.  Toddler sizes run 1-13.5.  The bigger child sizes then start at a size 1 again and go up to size 5.

Question 2

How do I measure the width? There is no bar to hold against the kid's food, it is too wide.

Hello and thanks for your question.  
Instead of using a width t-bar (as in the adult and T-bar Junior model), the US Junior device uses a slide to measure the width.  Position the slide so the arrows point to the size determined from toe or arch length.  Then find the width line which aligns to the widest part of the foot.

Question 3

I bought this to measure my kiddos' feet, but the word "infant" is written on it. Did I buy the wrong one? (My kids are 5 and 2.5 yrs...not infants.)

Hello, it is the Junior device for kids up to about age 10.  Don't worry, it begins with infant sizing and measures up to big kids size 5.
