Because Goku black got the Dragonballs and wished himself to be Goku knowing that he was the most powerful Saiyan that he thought that he was immortal and imuned to any attacks that anyone could dish out but in the alternative universe it really didn't fazed him even though it temporary caused him some pain in that fight with Goku in the future but due to the real Goku dying from the heart virus then being wished back that after that he no longer had the heart virus going into the future, I hope that answered your question considering that's all I do now is research,design and create Dragonball drawing along with playing every Dragonball game that ever came out plus all the series I keep watching over and over.
