Q: How do you get the information on the QR Code?

Use your smartphone to scan' the code found on the back of the ID. If you have not yet entered your personal data, you will be prompted to do so; if your data has been entered, you can edit, if necessary. Emergency responders can scan the code to access your medical data. Just an 'aside' ... it was REALLY HARD get the ID on my wristband!
