Q: The product name says it's a ball bearing fan, but the photos and specs show a brushless fan. Which is it?

All modern fans are brushless as far as I know... you might be thinking of sleeve bearing vs. ball bearing? But like it's title it's a brushless ball bearing 5015.

Q: How noisy is it? I want it to replace the blower on a humidifier I use at night and it has to be close to 0 noise

The fan is noisy I replaced the fan on my 3D printer at full speed.

Q: Can you adjust the speed of the fan?

You have to add a capacitor inline to control the speed of the fan. A 220uf 16v cap. If you look down at my review I have pics and explain it. It's extremely quick, cheap, and easy mod to do, but you need a soldering iron and a little know-how. (And the capacitor)
