Can someone who's had is chair for several months comment on it's longevity? Is it still holding up well?

Chair lasted 5 months.. then seat plate separated from cheap plywood seat and threw me to the floor. I weigh less than 250lb and it imploded.

What is the highest seat adjustment? I need 21.5" from floor to top of seat.

The highest the top of the seat goes is 20 1/2" away from the floor.

Does the chair tilt forward and if so is there a locking option to make it not tilt?
