Q: Do these have a hole that runs the length of the stem?

The hole runs through the yellow stem for the string which can be removed.

Q: How can i know if it will fit with my ear ? Is there any guidelines for sizing ? Also is it comfortable for sleeping ?

I believe it fits most adult ears as the earplug is tapered. I have small ears and they fit fine. I am also a side sleeper and I have no problem sleeping on it.

Q: can kids use these 7yr. and 9yr old?

depends on the size of their ear canals, I guess. the earplugs are conical, about 1/4 inch at narrow end, about 1/2 inch at big end(the outside), about 1/2 inch deep from top of cone to tip. there's a little yellow post that makes it fairly easy(for me anyway) to remove the earplugs. probably important not to force them into any ear canal. i guess it's best to contact the manufacturer for his recommendations, as i have no idea how kid's ear canals are, although i'm thinking the earseals could be too big for your kids.

Q: Has anyone tested these on their small breed dog? Neighbor keeps startling her with metal noises throughout the night?

I have visions of your dog choking on the plugs if there were to fall out,, best bet would be to check with your Vet. For humans these plugs work exceptionally well.
