What is the COO (Country of Origin)?

I suspect the mirror is actually manufactured in China (and, it would appear, the hook as well based upon another answer to this question) and placed into a box - that is, assembled - in the US. Not to take away from Sunshine Industries and their work with the disabilities, but manufacturing and assembling are not the same.

The tile in my bathroom is not perfectly smooth - it is textured - similar to the texture of natural stone. Will the adhesive still work in this case?

Absolutely. There's nothing magical about the adhesive. If you can find a relatively smooth spot maybe an inch square you're fine. And if it doesn't stick well you can reapply with a different adhesive strip. The mirror is light plastic so it won't be taxed by its weight.

Is the reflected image comparable to a glass mirror?

I would say as close as it can. Keep in mind that any mirror in the shower will collect water drops and will require some wipe down. I have had this for several months and wish I had known about it sooner. This is a no-brainer when it comes to travel and still wanting to shave in the shower.

How big is the actual mirror?

About 4" x 6", which is large enough for me to shave in the shower without any issues. Just make sure to run both sides under the water if you're taking a hot shower first otherwise it'll fog up.
