Why is Mine not working

It could be a bad tube or the amp itself. We are more than happy to set up a return if the tube is bad, but we can not verify without testing the tube. We suggest consulting a professional repair tech to verify if the amp is the problem or if the tube is bad. If they conclude that the tube is the issue, get in touch with us to set up a return/exchange.

What are the shipping dimensions of this item?

From a web search ... the "mechanical" (NOT shipping) dimensions of the tube are below. However, shipping dimensions would vary depending upon how the supplier decides to package the tube for shipment. It has been my experience that this is inconsistent and nearly impossible to predict. Suppliers typically package based on what will be sufficient to protect the item in transit and what materials they happen to have on hand. So, you might order from the same company 2 times and the shipping materials could be different. Would there be further volume reduction if I also replaced the second preamp tube?

Since a 12AT7 has only 60% of the gain of a 12AX7 yes it would reduce preamp output and volume at that stage.

I have this in a Zenith C845 AM/FM radio for over six years and so far it has worked and held up just like an NOS 12AT7. I have had no problems with it.
