Q: How do the sizes run? i'm 5' 9" 250 lbs, 2xl in us sizes, 3xl in eu. my irish rugby a few years was too small at 2xl.

My husband says that it runs a little bit on the tight side. He said if he got it again he would get a 3X. I hope this helps.

Q: real rubber buttons? or not

They seem to be, but they're not quite as flexible as buttons on some rugby shirts. They're certainly not hard plastic, though.

My husband is 240 lbs and I got him a 2XL and it fits him generously so you should have no problem. I got him an XL in another style and it also fits but less generously. Well made shirt so go for it!
