Does this actually keep the water heated while you clean?

No, it does not have a heating unit, but l have found that for even larger cleaning areas (3x3 feet) hot tap water (typically around 120 F) is sufficiently hot to do the job.

Do you always need to use a special solution or can you use just water or another type of solution?

The special solution is good to use for carpets. I would recommend using different solution for less aggressive cleaning. For example I sprayed shout to spot clean my mattress then fill the machine with water which I used to removed the dirt and solution from the mattress. Always READ the Warnings on what kind of surfaces cleaning solutions can or cannot be used on

Does this product kill dust mites?

I don't know. But I don't know how it could since it isn't hot or steamed at all.

The info says it works with 220 V, is that true?

I'm in the continental United States so my machine is set up for 120 volts(AC), and cannot be operated on 220 volts. Machines sold into the European and Asian markets may be set up for higher voltages though.
