Carpal tunnul braces with heat?

Just a brace with an incorporated metal brace. No heat other than the insulating effect of the material it is constructed of.

Is the thumb hold bigger in the larger one than the smaller one?

Since I only bought one, I would have no way of knowing that.

What is that extra foamy insert for?

I don't remember receiving a foamy insert with mine. I would speculate that you install it on the inside/backside of the brace to sandwich your wrist a more rigid brace.
