Pharmacy H&P Notebook Medical History and Physical notebook, 100 medical templates with perforations

This notebook is built well and has a plastic cover to protect the pages inside. The page that comes with is very useful and has comprehensive information. I did not realize it also came with badge attachments that have similar information to the sheet that comes with it. This is very helpful to always have important information with you when you are on the move. I will definitely be purchasing this again when I fill this notebook up!

I recently purchased this book because a friend recommended it. Once I got it in the mail, I was not disappointed. This is perfect for a pharmacy student going on APPEs. The only thing I wish I did was buy it sooner. The other soap notebook I got does not come close to comparing to this one! Buy now!

this notebook is super helpful for carrying on rounds for rotations, and keeps me off my laptop/iPad. The badge cards that come with it are super helpful as well when needing to quickly reference lab values. Was a little disappointed that the ID coverage badge card wasn't included in this set, but overall this has been so helpful!

I ordered the pharmacy H&P book, and was very excited as I’m starting my APPEs and thought this would help me keep up with my patients. It is good for that, and I love the book. However, the first time I ordered, I didn’t receive the badge reel guides, AND they sent me the quick guide for the OBGYN book. I returned it and had a replacement sent. When I got the new one it had the badge reel guides this time, but I still got the OBGYN quick guide. I was really hoping for the guide with the antibiotics and med conversions like in the picture, since I deal with A LOT of that, but at least the OBGYN one still has normal labs and stuff. Overall I will still get use out of the book and badge guides at least, so I can’t be too upset with (2/3).

Great buy

I am very pleased with this purchase. The size of the notebook is perfect to carry around but also big enough to have room to right. It came with a few little resource cards to attach to my badge which was a great surprise! I will definitely be using this for APPE rotations.

This was exactly what I needed as a 4th year pharmacy on my first rotation in a hospital. It made organizing all of the information into something manageable.

Very useful and practice for rounds, and it fits in your white coat. Allows you to gather all info needed to present during rounds
