Is the lever / hinge metal? Is it sturdy?

No it is not sturdy and it is not metal. The spout is metal but the mechanism is plastic. It works off a very ingeneous spring loaded PLASTIC pin. The issue is the housing it sits in is also plastic. Ours broke with weekly use within three months. I am trying to figure out how to contact seller for replacement part. That said...we love the machine and it has plenty of power.

Can I use grapefruit to juice, because the cap seems to fit only for orange?.

Yes!!! Its why we bought this one instead of the industrial one that only fits oranges. Sometimes you have to move the grapefruit half around to get all the juice...but not much. It works fantastic! Super easy to clean too!

It's the watts ..one has 160watts...other 120 watts ..if ur going to use it heavily you need a more power wattage . I purchased the model 3500 because I juice heavily

This is an excellent juicer for citrus, but not effective for pomegranate. My husband who uses this said it would be much more beneficial to just take the seeds out of the pomegranate and juice them in a regular juicer. We use the Hamilton Beach Big Mouth juicer, but any regular juicer should suffice. Hopefully, this is helpful to you... : )
