Package came folded in 3 places and Virgin Mary head fell off

Our Lady of Gaudalupe embroidery sequins came folded in 3. It was so hard to stretch it and add into a garment. I got it added into the garment and the client called me complaining that an hour later, after I dropped off the garment, the head of our Lady Guadalupe fell off.
