Hello, what you receive is just as the handle pictured. Maybe the angle pictured makes it look different.

Q: does this fit a sankey keg without the spear? thanks

No. This is much larger and oval; not round like a Sankey. They do make conversions to use your keg like a corney with ball locks in the lid from a 2” Sanitary fitting, if that’s what you are looking to do. It costs about $50 though.
