This item is a piece of crap. The first time I used it, following directions to the letter, the outer nylon got a burn hole and little beads ran everywhere. Because it is a "health" item, you can't return it. Seriously, don't waste your money.

Q: Does it work well with cold? Does it need to stay in the freezer?

It does not need to stay in the freezer. Only if you want to use it cold. If you want it hot put in your microwave. For me it did not stay hot enough for me. I never did try it yet for cold. I think it needs more stuff in it to keep it hot. The good thing about it is that you can put it on your shoulders and walk or sit with it and it does not fall off.

Q: How long do you put it in the microwave?

I don't remember but probably 1.5 to 2 minutes. This was one of the worst products I ever bought. I returned it. Whatever it was filled with was very uncomfortable to lean back on....sharp & poky feeling....and all the stuff shifted down into the quilted areas & did not provide even heat....and it did not stay warm for very long. It was a total waste of money.

Q: How do I use it for cold theraphy?

Put it in the freezer inside a plastic bag. Pull it out and put it on your shoulders.
