This box fits a 70mm ball. A 70cm ball will be too big for this 3.5 inch box.

Do these hold a standard sized cupcake with a frosting swirl?

Yes, boxes will hold standard or larger cupcakes. There are adjustable tabs on the insert based on cupcake size. There is also plenty of room for frosting and topper.

Does the 60 count come as 60 boxes plus inserts, or 30 boxes and 30 inserts?

The quantity of the cupcake inserts is euqal to the cupcake boxes. Hence, the 60 count comes as 60 single cupcake boxes plus 60 inserts.
