Does it require batteries? How many and how long would they last on average?

I think it comes with batteries. Was nearly a year ago I used it. It still works now, so battery life is good.

Do you get vibrations on all 4 electrodes or just the ones attached to the black leads?

Hi G Harris. I'm using the gadget right now. On the middle you can find a Mode button, it seems that it alters between channels. When it shows "2" in a circle on the right next to the "BURST" sign then it means both channels are activated (all 4 electrodes). If you press on the mode button again it changes to "1" in a circle then it feels that only one channel is working (2 electrodes). Hope it helped

When you press the boost button for a contraction, do you have to keep your finger pressed down on the button throughout the contraction?

No. You click the button once and it stays on until you click the button a second time when it will switch off. Once on leave on for the duration of the contraction.
