Q: Is this cable “wall plate to wall plate” or “equipment to wall plate”?

It's a fiber optic patch cable - typically these do not connect to a "wall plate." These are normally used for server to switch connectivity and in some cases switch to patch panel or patch panel to server.

Q: can the fiber be swapped to create a crossover to connect two computers directly without a switch?

The fiber would have to be crossed anyway, (transmit/receive) on each card. So yes, it should work :)
