how do you purchase replacement smoothing tips?

It comes with at least 3 tips, you would not need a replacement for a long while.

Will this work with Polyethylene?

The staples will work if the base material is at least 1/8 inch in thickness. The kit does include tips for use with plastic welding rod ( available on Amazon ) for thinner pieces but it will require a bit of practice. The staples require that the legs be nipped and ground down which requires very careful use of the grinder to keep from loosening. Also the repair on polyethylene will be difficult to be made cosmetically perfect

I’m not sure what the requirements are but you can plug it in your outlet or extension cord. I’m assuming 120V
