Exactly how bad is the wine/noise this card makes? I've read some reviews saying its really bad.

I have not heard it spool up. Playing at 2560x1600 with utra graphic settings in games like 'Ready or Not', 'Borderlands 3', 'The Last of Us', etc., my AIO makes more noise and it's just a noticeable increase in sound. I hope this helps

Since this card is OC do I need a cooling sytems (liquid or otherwise) just for it or will it be fine as is?

The card is built to run OC. Ive been running mine in a lian li 0c11 dynamic case just fine. No heat problems. I do not believe ASUS has produced any GPU AIO coolers for the 4090 cards. You could buy a MSI Suprim Liquid if you want an AIO cooled 4090.
