100% compatible with PurTest PT-12 from Home Depot
I can confirm that this is the (a) correct replacement kit for the PurTest PT-12 that is sold through Home Depot. The original bulb is labeled "Aqua Treatment Services ATS4-379" and the new one is labeled "GPH739T5L", and as far as I've been able to tell they're completely equivalent, both 33w, 4 pin, 739mm long, 253.7nm lamps. Obviously the PurTest PT-12 is a rebranded Aqua Treatment Services product.I ordered the combo kit because I wasn't sure if my quartz sleeve would be clear but I have a carefully maintained water softener upstream and it was fine. If you're sure your sleeve is OK just search for the bulb - "GPH739T5L" and it's even cheaper. But this package is so cheap compared to the actual ATS branded parts it doesn't hurt to order this and have a spare sleeve anyway.The only thing this doesn't include that might be nice is the o-ring at the top that seals between the sleeve and the UV appliance body. I wasn't able to easily find those here and just ordered a couple from Home Depot, they're listed as "544 Ultraviolet Light Disinfection O-Ring for PT 8, PT12, PT20 Lamps" and they're very cheap. Some food safe silicone grease may be helpful to get the o-ring and cap in place without damaging the ring. As best I can tell the part number for the o-ring is ATS8-544 but the one listing for it here right now shows unavailable.

Simple plug and play setup. Works as described as a replacement for factory bulb and glass.

Barely lasted a month...
I thought I'd save a bit of money with this product, but it lasted about 5 weeks before dimming and causing my system to alarm.

Had to buy combo
This is the exacts replacement bulb for my water sanitizer but the glass sleeve doesn't needto be replaced every year so wish I could just buy the bulb for a lot cheaper. Use to get it for30 bucks
