Is there a way to get a replacement ball (heat)?

Not sure, check with who you purchased from if you can. Second, email or call KT Tape on their website and check with them and see if they still make and sell them.

How long does it stay hot/cold?

I primarily use the hot and I would say it retains high heat for about 15-30 minutes

How in the world do you get the orange collar open? twisting and its not opening. ugh!

The orange collar snaps securely into place so getting it open may be a little tough to open up the first time. You may have more success flipping it upside down, gripping the orange part and turning the black portion as this gives you better leverage. Whichever side is up you'll be turning the collar/base counter-clockwise to open it.
