What is the measurement between back of flange and tabs? Is this for 3/8" or 1/2" (other) drywall?

Yikes. I don't recall tabs. I'm pretty sure they are universal. I'm almost certain they would be fine with either. I'm using them as air intakes for my engine on my boat. They are mounted in 3/16" Fiberglass. I did use an adhesive sealant as they are in my cockpit out in the elements.

Is the diameter of the hole required four inches or is the diameter of the grill four inches?:

I used a 4-1/8 cutter and it fit just fine. The actual dimension of the round sleeve that your vent tube slides onto is 3.7"
