These are printed with correct CDC info, but were not production-finished. My package of ICV arrived with multiple-sized, rough cut documents that I had to hand trim down to the printed cut-lines for it to fit into a passport...it would seem the finish trim step was skipped. When folded, after trimming, they should be roughly 125x90 millimeters -- the product I received was more like 150x210mm. Nothing a good straight edge and sharp razor knife didn't make short work of correcting the production mistake :)Granted, I bough them for family members to start documenting their vaccines in preparation of future travel...and recording YF vaccine, if ever needed for them; OTOH, if I were buying these for clinical use, such giving out to patients at a travel health clinic, they would have quickly been shipped back to Amazon for another batch.

You get what you pay for, and these are not authentic; obviously copied, not printed by the US Govt. Printing Office. I sent them back.

長期の海外滞在からの帰国後、今年の４月から海外渡航者の予防接種外来のセンター長をさせて頂いている。海外渡航者に持参させるワクチン接種証明書のフォームは決まっていないが、渡航先の医療機関がその証明書を見た時に、証明書の保有者がどのようなワクチンをいつしているか、すぐ分かるものがベストである。そのため、海外旅行者ワクチンを実施している医療機関は、そのような良い証明書を作るのに、それぞれ工夫努力している。この証明書はWHOに承認されたもので２５セットで売られている。英語とフランス語で書かれ、実施日、ワクチンの種類、量、医師のサインの他に、禁忌のワクチン、現在治療中の病気、薬剤の種類、量、医師のサインなど記入できるようになっている。色は目立つ黄色で、大きさも丁度日本のパスポートに収まるくらいである。赴任先のクリニックでは既にフォームがすでにできていたので、今回は採用していないが、ワクチン証明書としてそのまま使えるものである。既存のフォームと比較し、このWHO承認のものが使い勝手が良いなら、今後そっくりこのフォームに切り変えても良いくらいの完成度である。
