This Certificate is the W.H.O. (World Health Organization) bilingual version of the International Certificate of Immunization and Prophylaxis. The text is in both English and French. This includes 1 booklet with 1 sturdy Vinyl Cover large enough to hold your ICV booklet your Passport. Also known popularly as the Carte Jaune or Yellow Card, this is your record of routine and travel vaccinations to prevent disease (prophylaxis). It provides official proof of Yellow Fever Vaccination for International travel. Additional pages have space for your other vaccinations. The Yellow Card is recognized internationally and may be required for entry to certain countries where there are increased health risks for travelers. Before departing for a foreign destination, a traveler must visit a healthcare provider and receive the required vaccinations, which the doctor must list on the form per the instructions and sign and date. The Yellow Card must then be kept with the traveler's passport as he or she travels, since it serves as a medical "passport" of sorts as proof of vaccination, including booster shots. Reliable tracker of international vaccines
Ran out of room in my initial yellow book, and surprised that the new one was exactly the same. No need to fix anything that isn't broke, I guess!

This is the official vaccination record booklet from the WHO. The paper quality is pretty good, thicker than I thought it would be. The booklet is approx. 3 3/8 x 4 7/8 inch or 8.5 x 12.5 cm. I bought this from Good to Go Global. It came with a vinyl booklet holder (4 1/4 x 7 inch) is not like a bookcover for the booklet and more like a pouch, by the way. Pages 4-5 and 6-7 are each for 'International Certificate of vaccianation or prohpylaxis' and pages 8-9 and 10-11 are for 'Other vaccinations'. It's bilingual with both English and French. It will be handy for traveling.

arrived early and in great condition!
Going to the Philippines in less than a week and heard rumors about the IATF not accepting standard vaccination card/qr code so we got these for safety measures. It came early and is just as promised. Expensive but so worth it thank you!

Get all your immunizations on one card
Though in concept, its a great idea to have all your immunization records combined on to one card... I can only offer you "good luck" in getting it done. My PCP has taken the position that only the person or group that administered the shot or series is qualified to make these notations on another card. As if Doctors can't act for the good of their clients... Best wishes with our ever so fearful system...

Misleading Product Description - Only Received One Booklet Instead of 50
The description for this product clearly states that it is sold in sets of 50 booklets. However, I only received a single booklet, which is highly misleading and not what I expected based on the provided information. The product itself is fine and matches the WHO format, but the incorrect quantity makes this purchase very disappointing. If you only intend to sell single booklets, the description must be updated to avoid confusion. I do not recommend this product unless the seller corrects the misleading information.
