Q: does it have inside pocket?

No - there is no inside pocket. The snap buttons are also crappy and broke the first time I snapped them closed and then tried to pull them apart again. Don't buy this vest - go for something better.

I am 6' 235 and I got 2x, it fits just right, not tight, room to move around in it, my normal size is xl just to give you an idea of the sizes, hope this helped

If he has a 51 inches chest I would go with a 1xl vest to give him more roomI order a 1xl and I have a 56 inches chest it was tight so I sent it back and Got a1xl fits me great

It seems to me it is a small XL but for a medium it should be an okay fit. No problem with a 17 x 17.5 patch
