Are there two different sizes, or does it automatically come in the 2.25 inch size? I need it to fit my duty belt.

This is a Zak Tool model 55 which is for 2.25" belts. The Zak Tool 54 is the smaller size 1.75"

I want to hang some keys to the side of overalls, not on the belt but the top of the material. Will this work?

The loop is pretty tight. You may have trouble passing anything through it that's thicker then a 1/8". Otherwise it will work, you'll like how securely it holds onto the key ring.
