Many reviewers said they had thick hair. Will it fall out of medium thick, medium textured hair?

These are very high quality with good spring and closure mechanisms. I have thick hair but would think it would be fine with medium hair because it will close tightly on the available volume of hair.

I now have 4 barretts and can't figure out how to open them??

On the metal part, underneath the barrette, one of the ends has the opening mechanism. Just place your fingers on both sides of the metal part and squeeze inwards. It only opens from one end of the barrette, so if the end you try doesn't open it, then it will be on the other end.
